Anaesthesia in elderly patients poses significant challenges due to age-related physiological changes, including reduced cardiovascular reserve and altered drug metabolism, which increase the risk of haemodynamic instability and prolonged recovery. 
 

Propofol has long been the standard agent for general anaesthesia, but remimazolam, an ultra-short-acting benzodiazepine, offers a potential alternative with a different safety and efficacy profile.
 
This clinical review examines the findings of a recent meta-analysis comparing propofol and remimazolam in elderly patients, focusing on key indicators such as haemodynamic stability, induction choice, and long-term cognitive outcomes. The insights gained aim to inform clinical decision-making and optimise anaesthetic management in this vulnerable patient population.

3. Clinical Decision-Making: Providing insights into the factors influencing the choice between propofol and remimazolam for anaesthesia in elderly patients.
